The SIM is PUK locked. Unless you can find the PUK, it is wasted. The only remedy your mobile phone operator can help you is change your SIM (with the same parameter such as HLR provision, if they offer such) so that you can retain your old number. However, the entries saved in your old SIM card is lost. You can not retrieve it. entries such as phonebook and SMS. If you have saved these entries on your phone only, then they are safe, you would still recognize these numbers when they call you.
You will need to call up your phone network and ask them for it. Have some ID ready.
The PUK (pin unlock key) code is usually printed on the Back of the SIM pack check it out and if it isn’t there then u need to call your carrier and they will give to to you after verification.
